TLay Healthcare Services, Inc. was incorporated on April 24, 2006 as a registered home health agency serving four Northeast Florida counties – St. Johns, Duval, Clay, Flagler. As of June 2019 we added Volusia to our coverage area. – with the main office located in historic St. Augustine, Florida.

TLay Healthcare Services, Inc. strives to be one of the leading providers of a wide range of quality home health care services, recognized for enduring dedication to provide innovative, professional, and compassionate care to the communities we serve.
Dr. Imelda Nwoga is the Administrator of TLay Healthcare Services, Inc. since taking ownership in 2007. Striving to be a leading provider of home health care, Imelda has experience providing holistic care to clients and has been influential in planning, developing, and directing the operations at the agency.
Her academic preparation includes a doctorate degree in Community Health Nursing with a minor in Medical Anthropology from the University of Florida. She holds two Masters degrees in Curriculum and Instruction with an emphasis in Nursing Education from the University of Wisconsin and in Community Heath Nursing from D’Youville College. Her Bachelor’s degree was earned in Nursing from the University of Wisconsin in Madison.
As Administrator, Dr. Nwoga hopes to apply her knowledge and over 25 years of teaching and leadership experience to promote safe, timely, and quality client care at TLay. She would like to be a role model to other healthcare providers in fostering professionalism and excellence within the health profession.
