Job Title: Home Health Aides/ Certified Nursing Assistant

Description: Provides personal care and assistance clients under the supervision of the nurse or physical therapist. Their duties may include, but not limited to: assisting client with dressing, bathing, eating and maintaining a clean environment. The CNA/ HHA works with clients to ensure a safe, comfortable home to meet and maintain their activities of daily living.
Qualification: At least 18 years of age; must show proof of training (HHA) or be licensed as a Nursing Assistant (CNA) and meet applicable federal and state regulation; own transportation; possess good communication skills and ability to follow directions.

Job Title: Registered Nurse

Description: Coordinates and documents care for a client by administering the initial assessment visit. Utilizing physician’s orders and professional skills, the RN develops and implements a care plan that meets the client’s specific needs and is in compliance with federal and state regulations.
Qualification: At least 18 years of age; must be a RN from an approved school of nursing and meets all applicable federal and state licensure requirement; demonstrates supervisory skill and ability to work under minimum supervision; must show ability to read, write, and speak English.

Job Title: Licensed Practical Nurse

Description: Provide direct client care and ensure an environment that allows for privacy, dignity and well being of all clients in a safe and secure environment. They are responsible for providing prescribed treatment and personal care services to clients under the direction of a nurse.
Qualification: At least 18 years of age; graduate of approved Practical Nursing program; must be a LPN in good standing and meet applicable federal and state regulations; demonstrates supervisory skill and ability to work under minimum supervision; must show ability to read, write, and speak English.

Job Title: Physical Therapist

Description: Develops plan of treatment and provides skilled physical therapy services to increase range of motion, strength, and endurance in accordance with physician orders. Manages services provided to meet clients specific needs and in compliance with federal and state regulations.
Qualification: Must be a PT from an approved school and meet federal and state licensure requirements; maintain current PT license in the state; at least 1 year of supervisory experience as a PT; able to read, write and speak English.

Job Title: Occupational Therapist

Description: Evaluates a client’s level of function to determine a care plan to improve, restore, or compensate for lost function. He or she will be responsible for the teaching of techniques to increase independence in activities of daily living.
Qualification: Must be an OT from an approved school and meet all applicable federal and state licensure requirements; maintain current license in the state as OT; at least 1 year of supervisory experience as an OT; demonstrate supervisory skill and ability to work under minimum supervision; must be able to read, write and speak English.

Job Title: Medical Social Worker

Description: Assesses, plans, and organizes a therapy program that provides social, environmental, and emotional counseling to the client. By utilizing resources, they help the client resume life in their community or to learn to live with a disability.
Qualification: Must hold a MSW degree from an approved school and meet all applicable federal and state licensure requirements; at least 1 year of supervisory experience as an MSW; demonstrate supervisory skill and ability to work under minimum supervision; must be able to read, write and speak English.

Job Title: Speech Therapist

Description: Evaluates a client’s level of verbal function and determines if a speech therapy program could improve or restore lost communication skills.
Qualification:Must be an ST from an approved school and meet all applicable federal and state licensure requirements; must be licensed with the state and possess a Certificate of Clinical Competence by the American Speech and Hearing Association; at least 1 year of supervisory experience as an ST; demonstrate supervisory skill and ability to work under minimum supervision; must be able to read, write and speak English.
